HeadStart Kent – Family Focused Transition

Service: Through HeadStart Kent, Salus are able to offer a Family Focused Transition service to children and young people in Swale and Gravesham.

Aim: The aim of the programme is to improve the emotional well-being and resilience of children and young people through or following transition.

Who is it for:

  • The service is available to young people in the year 6 transition period or young people who have found the transition difficult and attend schools engaged in the programme in Swale and Gravesham (please click on the area¬†to view the current¬†list).
  • Reasons for referral may include general adversity and other family issues which affect attendance, attainment and/or behaviour.
  • Participants of the service will include family groups.
  • Young people who are referred to the service may have a diagnosis such as ADHD, ASD, conduct disorder or significant behavioural issues.

The offer: Salus adopts a strengths based, future focused approach to supporting children, young people and their families. Using a range of evidence based approaches and tools and through a team of talented transition mentors, we will work to understand the need and develop a programme to address this. Following assessment of need, the mentor will offer approximately 10 weeks of support. It is envisaged that this will include group based support for families and young people.
